Thrombosis & Bleeding
Simplified PESI (sPESI) Calculator
Six-item version of the PESI that separates low-risk from higher-risk acute pulmonary embolism.
Formula
1 point each:
Age >80 years
Cancer
Chronic cardiopulmonary disease (heart failure or chronic lung disease)
Heart rate ≥110/min
Systolic BP <100 mm Hg
Arterial O₂ saturation <90%
sPESI 0 = low risk; ≥1 = high risk
How it is used
- sPESI 0 is low-risk PE when there is no RV dysfunction on echo or CT and no raised troponin (if checked): carefully selected patients can be considered for early discharge and home anticoagulation (ESC 2019).
- sPESI ≥1 is at least intermediate risk: check the RV and troponin to separate intermediate-low from intermediate-high risk (ESC 2019).
- As accurate as the full PESI; 30.7% of derivation and 36.2% of validation patients scored 0 (Jiménez 2010).
- For confirmed acute PE only; shock or persistent hypotension is high-risk PE whatever the score (ESC 2019).
Key points
- A score of 0 does not outweigh RV dysfunction, a raised troponin or clinical deterioration: such patients are intermediate risk (ESC 2019).
