Mechanism of action

Phytonadione (vitamin K1) is the cofactor for γ-glutamyl carboxylase in the liver's endoplasmic reticulum. In its reduced (hydroquinone) form it lets the enzyme carboxylate glutamate residues in the Gla domains of factors II, VII, IX and X and proteins C, S and Z, and it is oxidized to vitamin K epoxide. The γ-carboxyglutamate residues bind calcium, so the factors can attach to phosphatidylserine on activated platelets. Warfarin blocks VKORC1, which recycles the epoxide; large doses bypass the block because a warfarin-insensitive reductase (FSP1) reduces vitamin K directly.

Pharmacology

Pharmacokinetics

OnsetIV 1–2 h; oral 6–10 h (labels); SC slow and erratic
PeakIV: bleeding controlled in 3–6 h, INR often normal by 12–14 h (label); oral matches IV by 24 h (Lubetsky 2003)
Half-lifeIV: 22 min, then about 2 h (DrugBank)
DistributionFat-soluble, carried in lipoproteins; Vd about 20 L (DrugBank); small stores; little crosses the placenta
MetabolismLiver: epoxide cycle; side chain shortened, then glucuronidated
EliminationBile and urine as metabolites; almost none unchanged (label)
BioavailabilityOral absorption needs bile salts and runs through intestinal lymph
