Scale from -5 (Unarousable) to +4 (Combative).

How it is used

• Goal RASS for most mechanically ventilated patients is 0 to -2 (Light Sedation).
• Deep sedation (-4 to -5) is generally reserved for severe ARDS, status epilepticus, or elevated ICP.
