Anticoagulation, endocrine and GI
Eptifibatide (Integrilin) drip
Integrilin, GP IIb/IIIa inhibitor
Dosing
| Start | 180 mcg/kg IV bolus from the 2 mg/mL vial, then 2 mcg/kg/min; PCI adds a second 180 mcg/kg bolus 10 minutes after the first. CrCl under 50 mL/min (Cockcroft-Gault): same bolus, then 1 mcg/kg/min |
|---|---|
| Titrate | Fixed rate, not titrated. ACS: up to 72 hours (96 in all if PCI follows); after PCI: 18–24 hours (at least 12) |
| Maximum | Over 121 kg: bolus 22.6 mg and infusion 15 mg/h, or 7.5 mg/h with CrCl under 50 (label). Now mainly a bailout during PCI for large thrombus, no-reflow or slow flow (ACC/AHA 2025) |
| Common bags | 75 mg in 100 mL (750 mcg/mL); 200 mg in 100 mL (2 mg/mL) |
| Dose units | mcg/kg/min |
Line and administration
- Infusion runs undiluted from the 0.75 mg/mL vial or premixed bag (or the 2 mg/mL 100-mL vial)
- Can share a line with heparin, nitroglycerin, morphine, midazolam or metoprolol; not with furosemide
Watch for
- Bleeding, mostly at the arterial access site: aPTT 50–70 s on heparin (ACT 200–300 s at PCI); pull the sheath with heparin off 3–4 hours and aPTT under 45 s
- Platelets: an acute immune drop can come within hours; stop eptifibatide and heparin if under 100,000
- Contraindicated on dialysis; platelet function recovers within about 4–8 hours of stopping; no antidote
